New students should return their required forms before August 1. Returning students should return all required forms to Health Services after August 15. Use mailing address or fax number at right. Required forms are in bold, followed by an asterisk.

For New Students & Parents

Explanation of Required Health Information ~ 116K

CareFlow* CareFlow, our electronic student health record, must be completed on all new students. A link will be available in the right sidebar on Parent Central under "Secure Links" by July 1. Please note that you cannot see secure links unless you are logged in. Your login information will be mailed during the last week in June. Please log on at your earliest convenience to complete the form, but no later than August 1.

Every Hotchkiss student is required to have insurance coverage. If you do not carry your own policy, click here for information about the plan offered through the School.
